Liana Freires, FNP-C — Family Nurse Practitioner
Liana Freires is a family nurse practitioner caring for adults and children at Pickens Urgent Care & Primary Care. She provides care for everyday illnesses, chronic conditions, wellness visits, and preventive health needs. Her approach begins with listening and developing treatment plans tailored to each patient.
She earned her Master of Science in Nursing as a Family Nurse Practitioner from Kennesaw State University and completed additional training in adult-gerontology acute care at Georgia State University. Her experience includes family medicine, pediatrics, urgent care, and hormone therapy. When appropriate, she also discusses hormone replacement therapy following an individualized evaluation.
She speaks English, Portuguese, and Spanish, helping patients and families feel comfortable discussing their health. Whether caring for a child, managing a chronic condition, or supporting a patient through menopause, her goal is to provide thoughtful care and clear guidance in a welcoming environment.

Jo Snodgrass — Family Medicine Nurse Practitioner
Jo has been working in healthcare since 2010, beginning her career in a hospital setting. Her passion for helping others and building meaningful connections with patients has continued to grow throughout her career. In 2022, she had the opportunity to meet Dr. Vazquez, and in 2023, she joined the team at Hope and Health Clinic.
She truly enjoys working with people and is especially passionate about preventive medicine and helping patients take an active role in their health and well-being.
Outside of work, Jo enjoys spending time with her two sons and extended family. She loves traveling, exploring new places, and learning about different cultures. She also enjoys reading and making time for the people she loves. Being one of seven children, family has always been an important part of her life.
